Here is the kit I am going to go with on my 64.

DBK6272A-40 - 1962-1972 Mopar A Body Small Bolt Pattern Standard Disc Brake Conversion Kit (at https://piratejack.net/dbk6272a-40/ )

You will need to upgrade your wheels to something bigger, should you go with a disc brake upgrade.

You will also need the master cylinder, combination valve and associated steel brake lines to go with it. The PJ kit just shows the front brake hardware. It is incomplete without the disc/drum hydraulics.
